Co-signer documents

Type in your information, print, sign, and send these back to Angie. If you're not sure which ones apply to you, call and we'll tell you.

How it works

  1. Download and print

Download all three forms below and open them in Adobe Reader, Chrome or Edge. Click any line to type your answers, then print on regular letter-size paper (8.5 x 11) at 100% size.

  2. Complete the yellow areas, then sign

On each form, the yellow highlighted areas are for the co-signer to fill in or sign. Use blue or black ink. The blue area marked OFFICE is completed by Angie Hale Bail Bonds, so leave it blank. Signature lines are signed by hand in ink after printing.

  3. Send it back

Email a clear scan or photo of each signed page, or drop the paper copies at the office. See the options below.

Call first

Not sure what to sign or how to send it? Call and we'll walk you through it.

Online signing is coming. Angie is working on letting co-signers sign from their phone, so no printer is needed. Until then, print-and-return is how it works.

Once your paperwork is signed and you know your payment amount, you can pay on the payments page.

Paperwork can include personal details. Regular email is not private, so please use the secure upload above, or call or drop it off instead.
